- Assess patient's condition.
- Gather all the equipments.
- Explains the procedure to your client.
- Position client on sitting or lying position.
- Wear gloves
- Empties the pouch when 1/3 to 1/2 full and assess for consistency and amount of affluent.
- Removes appliances and discard.
- Using warm water and milk soap cleans the peristomal skin and the stoma.
- Dries the area thoroughly by patting with towel.
- Check the stoma and peristomal skin.
- Place a piece of tissue or gauze pad over the stoma, and change it as needed.
- Measure the size of the stoma. Make an opening in the skin barrier. Remove backing to expose the sticky adhesive side.
- Removes the tissue or gauze pad on the stoma.
- Center the skin barrier over the stoma and gently press it onto the skin, smoothing out wrinkles or bubbles.
- Removes the air from the pouch.
- Documents for the characteristics of the stoma.
