Colostomy Procedure

  1. Assess patient's condition.
  2. Gather all the equipments.
  3. Explains the procedure to your client.
  4. Position client on sitting or lying position.
  5. Wear gloves
  6. Empties the pouch when 1/3 to 1/2 full and assess for consistency and amount of affluent.
  7. Removes appliances and discard.
  8. Using warm water and milk soap cleans the peristomal skin and the stoma.
  9. Dries the area thoroughly by patting with towel.
  10. Check the stoma and peristomal skin.
  11. Place a piece of tissue or gauze pad over the stoma, and change it as needed.
  12. Measure the size of the stoma. Make an opening in the skin barrier. Remove backing to expose the sticky adhesive side.
  13. Removes the tissue or gauze pad on the stoma.
  14. Center the skin barrier over the stoma and gently press it onto the skin, smoothing out wrinkles or bubbles.
  15. Removes the air from the pouch.
  16. Documents for the characteristics of the stoma.
